Warwickshire County Council is pleased to announce that we are looking for a full time Reablement Co-Ordinator (Supervisor) to join Warwickshire’s Reablement team. We are a well-established team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Co-Ordinators, Reablement Assistants, Social Workers, Social Care Practitioners and Administrators, who have provided years of interventions for Warwickshire Reablement customers and aim to achieve the best outcomes to them going through their Reablement journey.

Reablement focuses on promoting independence by supporting customers to do as much as possible for themselves both physically and psychologically. We do this by supporting customers to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home.

We believe that by supporting customers to identify and achieve their own personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on their own health and well-being.

The role

As a Reablement Co-Ordinator (Supervisor) your role will include line management of a small team of Warwickshire County Councils Reablement Assistants. You will ensure a safe and effective Reablement service is offered to the citizens of Warwickshire. Working in partnership with the Reablement Assessment Team and other professionals to provide an outcome focused service to customers which promotes independence and maximises daily living skills. You will be required to undertake, develop and manage work programmes for Reablement Assistants, with the aim of providing a consistent service to customers that is safe, high in quality and cost effective. You will manage the Reablement referral through their adult customer journey within Reablement. You will also ensure that needs and outcomes identified within the Reablement support plan are met. The post will be based either at Kings House, Bedworth or at Heathcote House, Leamington Spa including hybrid working from home after a satisfactory induction. The post will be for 37 hours per week. There will be an expectation that the post holder will be required to provide Warwickshire wide support to Warwickshire Reablement service.

Key Requirements

We are a fast-moving responsive service and are looking for dedicated individuals, with a passion for care provision, who want to make a positive difference for the people living in their local communities. You will be working within the Service to respond efficiently to referrals for social care and support. You will manage a small group of Reablement Assistants.
You will be responsible for managing your time and workload and will be expected to work closely with your colleagues within the team and external agencies /services, as well as independently.
Excellent communication skills and the ability to work within a team as well as on your own initiative is vital to this fast-paced role. Having good time management/organisation skills is required; being solutions focussed flexible in your approach, willing to travel across the county as needed, as well as having experience of handling confidential information with tact and discretion.

The post requires you to work a shift pattern of 7 am – 3 pm, 9 am – 5 pm and 2 pm – 10 pm Mondays to Fridays, one weekend in every 3 weeks. There will also be requirement to work bank holidays on a rota basis. You will receive enhanced payments for working unsocial hours on Saturday and Sunday and Bank holiday working. Your working hours will include hybrid working and the ability to work from home.

Having use of a car and a full driver’s licence is essential for this role.
